Check-list - I have a refugee status or a subsidiary protection or permission to stay in Poland for humanitarian reasons
Documents needed to obtain a stamp in your passport (so-called formal requirements):
• application in 1 copy, completed online in the Case Management Module or in accordance with the instructions ( application to download ).
• 4 photos ,
• A photocopy of a valid travel document (the original is available for inspection). In particularly justified cases, if you do not have a valid travel document and are unable to obtain one, you may present another document confirming your identity.
To initiate the proceedings, proof of payment of the stamp duty is also required.

• a decision confirming that you hold the refugee status, subsidiary protection, or a permit to stay for humanitarian reasons,
• documents confirming that your stay in the territory of Poland was uninterrupted or documents indicating the reasons for interruptions in the stay.
During the proceedings, you may be asked to come to the office for an interview to provide additional clarifications.
