A stay is uninterrupted if the one-time stay outside Poland was not longer than 6 months at a time and all stays outside Poland did not exceed 10 months in total. This means that your trips abroad during the specified period should not exceed the limits mentioned above. However, there are exceptions to this rule. If you are employed by a company based in Poland and your absence is work-related (a business trip) or due to accompanying a spouse/parent who works in Poland, such absence is considered justified.
Permanent residence
A document confirming affiliation with the Polish nation, issued by Polish consulates and voivodeship offices within the territory of the Republic of Poland.

A valid template can be found on our website describing the procedure. Submitting an unsigned or incomplete form or not using a valid official template is considered a formal deficiency.
These refer to documents without which we cannot initiate proceedings, i.e., we cannot consider your application — for example, a completed and signed application form, photographs, the original passport presented for inspection, etc. You must provide these documents in order to receive a stamp on your passport. The formal assessment of the application is the initial stage of the entire procedure, during which we review the documents you have submitted.
