Insurance that covers medical treatment costs in Poland; it allows you to visit a general practitioner and receive hospital care. It is not accident insurance (so-called, NNW). This refers to insurance that enables you to use both outpatient (clinic) and inpatient (hospital) medical services, or a current insurance policy that confirms coverage of treatment costs by the insurer. A document confirming your insurance may include:
•    NFZ insurance agreement
Submit the original insurance agreement with the National Health Fund (pol. NFZ) for inspection, along with the proof of paying the insurance contributions (from the date of conclusion of the agreement until the date the decision is issued).
Remember: each monthly contribution (premium) must be paid by the 15th of the following month!
•    Entitlement under social security coordination regulations
e.g., registration with the Social Insurance Institution ( pol. ZUS)
Provide confirmation of registration with ZUS and proof of paid contributions.
•    Foreign health insurance
If the insurance was issued outside Poland, it must be translated into Polish by a sworn translator. Submit both the original insurance document and the original translation.
•    Private health insurance.
The private health insurance must cover medical treatment costs in the territory of the Republic of Poland (i.e., you must include outpatient and hospital services). Accident insurance (NNW) is not sufficient to confirm that you have insurance to cover medical expenses. The private health insurance must be valid on the date of the decision.
If the insurance contribution is paid in instalments, attach the proof of payment (original for inspection).

Health insurance or travel medical insurance with a minimum coverage amount of EUR 30,000 is required for the visa extension procedure. The insurance must be valid for the entire duration of the planned stay in the territory of the Republic of Poland and must cover all expenses that may arise during your stay in Poland, including the expenses related to a return trip for medical reasons, the need for emergency medical assistance, urgent hospital treatment, or death.

The amount of fees related to your place of residence. They include fixed charges for using the occupied premises (e.g. rent), as well as utilities such as electricity, gas, water, sewage, and waste disposal. These costs should be calculated per person living on the premises. If you are studying in Poland and have submitted an application for temporary residence due to studies after April 27, 2019, remember that you must have sufficient financial resources to support yourself and cover your return trip home after deducting housing costs. Calculate how much you pay for your room, apartment, or dormitory, subtract that from your financial resources, and check whether you still have sufficient funds for subsistence and return to the country of origin.
