Polish Travel Document

A document issued to a foreigner who does not have a valid passport, for example, it has been lost or expired, and a new passport cannot be obtained (e.g., there is no embassy of their home country in Poland). You are entitled to obtain a Polish travel document, if you have been granted: a permanent residence permit, a residence permit for EU long-term resident, subsidiary protection, or permission to stay for humanitarian reasons. The Polish travel document is issued for 1 year and entitles the holder to cross borders during that time.
