This document confirms the registration of residence of an EU citizen in Poland. It is valid for an indefinite time.
EU citizens
The right to reside in specific residential premises where your life centre is located, encompassing your family, personal, and financial matters. While considering your application, you will be required to provide a document confirming secured accommodation in Poland.
The person who signed the application; in the case of minors, the applicant is the parent or legal guardian.
Depending on your place of residence, your application will be processed by the Voivode(Governor) of the respective province (voivodeship). The same principle applies to most institutions you will need to contact. If your employer submits an application for a work permit, the decision will be issued by the Voivode of the province where your employer’s company is based. If you are applying for an intra-corporate transfer, the application will be processed by the Voivode of the province where the host entity in Poland is located.
The office responsible for issuing birth certificate, marriage, and death certificates. At this office, one can register foreign civil status acts, change their first name or surname, get married, or acknowledge a child.
Insurance that covers medical treatment costs in Poland; it allows you to visit a general practitioner and receive hospital care. It is not accident insurance (so-called, NNW). This refers to insurance that enables you to use both outpatient (clinic) and inpatient (hospital) medical services, or a current insurance policy that confirms coverage of treatment costs by the insurer. The Schengen Area is a zone in Europe where internal border checks have been abolished. It currently includes 27 countries, among them almost all European Union member states except Ireland and Cyprus. Additionally, four non-EU countries are also part of the Schengen Area: Iceland, Norway, Switzerland, and Liechtenstein.
For EU citizens, a stay is considered uninterrupted if all absences from Poland do not exceed 6 months in total. This means that your trips abroad during the specified period should not exceed the limits mentioned above. However, there are exceptions to this rule. If you are doing military service or your absence is due to exceptional personal circumstances, such as pregnancy, childbirth, illness, or for reasons related to professional assignments, studies, or vocational training, and these absences do not exceed 12 months in total, include this information in your application.
A document confirming the right of permanent residence of an EU citizen, it confirms that the EU citizen has resided in Poland for a period of at least 5 years.
A document issued to a family member of an EU citizen who does not hold the citizenship of an EU Member State, confirming a shared residence and a common household with the EU citizen in Poland.
