It is an agreement concluded in writing and made before a public notary, in which one person/persons agree to regularly provide a certain amount of money in favour of another person.
Family
The guardian is appointed by the guardianship (family) court in cases of a child whose parents are unknown or who is in Poland without their parents and needs a guardian to represent their interests, for example, in dealings with authorities.
A current document issued by the civil registry office, not earlier than three months before the submission of the application, confirming that a marriage has been concluded. In cases where the marriage certificate was issued outside Poland, a certified translation of the document must also be submitted.
A person who according to the law is under the age of majority. In Poland it is every person under the age of 18.

A valid template can be found on our website describing the procedure. Submitting an unsigned or incomplete form or not using a valid official template is considered a formal deficiency.
An annotation to be found on the residence card if you are entitled to work in the territory of the Republic of Poland or you are exempt from the obligation to hold a work permit. This annotation does not mean full access to the Polish labour market. Your access may be limited, e.g. if you have received a temporary residence and work permit, you are allowed to work for a particular employer and in a specific position, as indicated in the decision.
A family member is defined as a spouse (i.e., wife or husband) and a child under the age of 18 i.e., a minor). Other relatives of the foreigner, such as siblings or parents, are not considered family members under this status.
A document issued by the civil registry office confirming the fact of birth. In case of documents issued outside of Poland, a certified translation by a sworn translator must also be submitted.
These refer to documents without which we cannot initiate proceedings, i.e., we cannot consider your application — for example, a completed and signed application form, photographs, the original passport presented for inspection, etc. You must provide these documents in order to receive a stamp on your passport. The formal assessment of the application is the initial stage of the entire procedure, during which we review the documents you have submitted.
