When to submit the application?

A permanent residence permit is granted to a person who has held a temporary residence permit for a victim of human trafficking for at least 1 year, has cooperated with law enforcement authorities, and has justified fears of returning to their country of origin.

Where should I apply?

1.    Go to mos.cudzoziemcy.gov.pl and create an account
2.    Read the information and list of the most common mistakes
3.    Complete the application for a temporary residence permit
4.    Please provide your email address in the application .
5.    Print the application form and sign it
6.    Send the printed form with attachments via postal operator to  the Opole Voivodeship Office, Department of Citizens' Affairs and Foreigners 
7.    or deliver it to
the Registry Office of the Opole Voivodeship Office (main hall of the Office) open from Monday to Friday during office hours, i.e. from 7:30 a.m. to 3:30 p.m.). 
 

What documents do I need?

Documents needed to obtain a stamp in your passport (so-called formal requirements):
•    application in 1 copy, completed online in the Case Management Module or in accordance with the instructions ( application to download ).
•    4  photos ,
•    A photocopy of a valid travel document (the original is available for inspection). In particularly justified cases, if you do not have a valid travel document and are unable to obtain one, you may present another document confirming your identity.

Documents required to be granted the permit:
•    documents confirming that you cooperated with law enforcement authorities and that you have justified fears of returning to your country of origin,
•    a copy of the decision granting a temporary residence permit for a victim of human trafficking.
 

To initiate the proceedings, a proof of payment of the stamp duty is required.

During the proceedings, you may be asked to come to the office for an interview to provide additional explanations

What should I know?

The permit is issued for an indefinite period. This permit entitles you to work without a work permit.
