When should I apply for the issuance of the document

You should submit an application for the issuance of a Polish identity document for a foreigner if:
•    you are staying in the territory of the Republic of Poland and you do not hold any nationality, if the interest of the Republic of Poland so requires;
•    you are a victim of human trafficking.
This application should be submitted for a minor foreigner born in the territory of the Republic of Poland and residing in that territory without parental care, if it is not contrary to the interests of the Republic of Poland and is in the best interests of the child. An application on behalf of a minor foreigner is submitted by guardians appointed by the court, or by one of the guardians appointed by the court.
 

When should I apply for the replacement of the document

Polish identity document for a foreigner should be replaced if:

  • the data contained in the existing document has changed, i.e., your name, or your address of residence;
  • your appearance has changed to such an extent that the photo included in your current document no longer allows for your identification;
  • you have lost the current document;
  • the current document has been damaged.

An application for the replacement of a Polish identity document should be submitted within 14 days of the occurrence of the above-mentioned circumstances. An application on behalf of a minor foreigner is submitted by the parents or guardians appointed by the court, or by one of the parents or guardians appointed by the court.

Where to submit the application

You may submit the application at the Correspondence Registry Office of the Opole Provincial Office.
If you are unable to submit the application in person, send the paper version of the application by post.

All documents are submitted in original / certified copy.

What documents do I need to provide with the application

•    An application for the issuance/replacement of a Polish identity document for a foreigner in 1 copy, completed in accordance with the instructions (download the application form);
•    A recent photograph;
•    A document confirming identity (you are exempt from the obligation to present a valid travel document or identity document if you hold a certificate stating that you are a victim of human trafficking and, due to exceptional circumstances, you do not have such a document and cannot obtain one);
•    Documents necessary to confirm the data and circumstances stated in the application (e.g., documents confirming the impossibility of obtaining a travel document from your country of origin, documents confirming statelessness, the decision granting subsidiary protection in the territory of Poland or a permit to stay for humanitarian reasons);
•    A confirmation of registered residencefor a permanent or temporary stay,
 

Fees for issuing / replacing a Polish ID

The pay rate for application submitted until 29th July:

Fee for issuing a Polish ID is 50 PLN

Reduced fee for issuing a Polish ID is 25 zł

You are entitled to reduced fee if you are:

  • student, or
  • student at upper secondary schools, or
  • a minor child up to 16 years old, or
  • in difficult financial situation.

Replacement of a Polish ID for a foreigner in the event of culpable loss or destruction is 100 PLN

Replacement of a Polish ID for a foreigner in the event of further culpable loss or destruction is 150 PLN 

The pay rate for application submitted from 29th July:

Fee for issuing a Polish ID is 100 PLN

Reduced fee for issuing a Polish ID is 50 zł

Replacement of a Polish ID for a foreigner in the event of culpable loss or destruction is 200 PLN

Replacement of a Polish ID for a foreigner in the event of further culpable loss or destruction is 300 PLN

What should I know?
 

The Polish identity document for a foreigner is issued by the voivode competent for your place of residence. If you are refused to be granted the Polish identity document, you will receive a decision. A Polish identity document for a foreigner is issued for a period of 1 year. It confirms only your identity, it does not confirm nationality, and does not entitle you to cross the border. This document does not legalise your stay in the territory of Poland, i.e., it is not the same as obtaining a visa, a temporary residence permit, a permanent residence permit, or a permit for residence of a long-term EU resident.
The document shall be collected in person at the Foreigners’ Service Centre of the Opole Provincial Office.
When collecting the document issued to a minor who has reached the age of 6, the personal appearance of the minor is required.

 

To collect your document, bring a valid travel document and the previous document if one was issued.
