PESEL number is an eleven-digit numeric identifier that enables easy identification of the person it is assigned to. PESEL number includes the date of birth, a serial number, gender designation, and a control digit. The PESEL number is commonly used in contacts with various institutions and in legal transactions, such as:
•    all types of official matters, e.g., marriage, change of residence, inheritance or court proceedings;
•    employment purposes, including employment contracts, contracts for specific work, or mandate contracts;
•    customer identification and access to certain public services, such as monthly public transportation tickets;
•    tax declarations for individuals not conducting business activity;
•    financial services, for example, opening a bank account or taking out a loan;
•    starting your own business, e.g., sole proprietorship or civil law partnership;
•    medical care for verifying whether a person has insurance or indicating the PESEL number on a medical leave note;
•    civil-law contracts, for example, buying or selling a car;
•    and others...

Leaving Poland for more than 30 days results in a loss of the UKR status and its automatic change to NUE status.
Do you want to check your PESEL UKR status? The easiest and fastest way to check your UKR status is by using the dedicated government website: https://www.gov.pl/web/ua
